All persons with diabetes benefit from diabetes educa tion as they learn self-management skills to control their diabetes and optimize their ability to live a productive and satisfying life. The Diabetes Education Program Offers: • Inpatient diabetes ducation services are provided by certified diabetes educators, nurses and dietitians, for your one-on-one individual needs. • Outpatient diabetes education services are provided on an individual basis, through group classes or a combination of both depending on your needs. • Our unique, internationally recognized group classes are held on a monthly basis and offer education in all aspects of diabetes management. During these class es, you will learn about healthy lifestyle changes, healthy nutrition, blood glucose monitoring and inter pretation, medications for diabetes and problem solving. Insurance Outpatient services may be reimbursable when your attendance is by physician referral. Your individual insurance policy defines the extent of your coverage. If you have any questions regarding insurance coverage, call the Diabetes Education Office at 216.761.4286. Community Support The Diabetes Management Program at Huron Hospital offers support services for your diabetes self-manage ment needs. • A Diabetes Support Group meets at various times throughout the year and includes refreshments and door prizes. There is no fee for this program. • The Community Health Center provides a wide range of health care services for people with diabetes, including ophthalmology and podiatry.

Project HEAL (Health, Education & Active Learning) • Multi-level education initiative that teaches girls of African descent to analyze, evaluate and make healthy lifestyle choices • Empowers students to gain knowledge, skills, and opportunities to become active health advocates for policy changes in their schools, families, and neighborhoods Make the Link!
